Background: Nivolumab is a human monoclonal antibody against programmed cell death receptor-1 (PD-1) capable of tumor rescue secretly infiltrating cytotoxic T lymphocytes (CTL) to recover their ability to kill target cells expressing the peptide epitopes specific tumor antigens derived bound homologous human leukocyte antigen (HLA) molecules. Nivolumab currently active therapeutic agents but expensive for metastatic non-small cell lung cancer (mNSCLC), producing, in some cases, the immune-related adverse events (irAEs). Currently, there are no reliable biomarkers have been validated to predict either treatment response or adverse events in patients treated.


Methods: We conducted a multi-institutional retrospective analysis included 119 patients with mNSCLC who receive PD-1 blockade since November 2015 to investigate the predictive role of germinal class I HLA and DRB1 genotypes. We investigated the correlation between patient outcomes and the frequency irAEs with specific HLA A, B, C, and DRB1 alleles with a specific sequence of DNA typing reverse oligonucleotide (SSO).


Results: A poor outcome in patients negative for expression of the two alleles of HLA-A is the most frequently detected (HLA: HLA-A * 01 or A * 02; progression-free survival (PFS): 7,5 (2,8- 12.2) vs 15.9 (0 to 39.2) months, p = 0.01).

In particular, patients with HLA-A * 01-positive indicates a prolonged PFS of 22.6 (10.2 to 35.0) and overall survival (OS) of 30.8 (7.7 to 53.9) months, respectively -masing. We also reported that the HLA-A and DRB1 locus heterozygosis (het) were correlated with OS het bad if we considered the locus of A; inversely, correlated with the length of survival in DRB1 het.


Conclusion: This study shows that class I and II HLA alleles characterization to determine the immunogenicity of tumors have relevant implications in predicting success in mNSCLC nivolumab and give reason for further prospective trials of cancer immunotherapy.

Class I Human Leukocyte Antigen Detection and Response to Platelet Transfusion Patients Undergoing Liver Transplantation

Background: Patients undergoing liver transplantation (LT) often receive transfusions of platelets (PLT) to minimize the risk of bleeding. Alloimmunization for platelets can cause refractoriness to PLT. Data on the implications of platelet alloimmunization in patients undergoing LT remains limited. We investigated the effect of human leukocyte antigen class I (HLA-I) antibody in PLT refractoriness and short-term outcomes after LT.


Methods: The clinical factors and PLT Peritransplant reviewed for all adult liver transplantation or simultaneous liver-kidney patients from 2012 to 2017. Sensitive (SE) with HLA-I calculated pretransplant panel-reactive antibody ≥20% compared to patients not sensitized (USA) with calculated panel-reactive antibodies <20%. Mean follow-up was 21.4 mo.


Results: alloimmunization observed in 39% of the study group. SE (n = 28) received 272 PLTS, and the US (n = 44) received 246 solar. History of pregnancy was higher in the SE of the US (P <0.01); if not, both groups had similar clinical characteristics. SE has a higher level of PLT refractory (66% versus 47%; P <0.01) from the US.

Mean platelet count corrected lower rise in the SE compared to the US until 100 minutes after the PLT (P <0.05). Conclusions: LT patients experiencing high levels of HLA-I and PLT refractory alloimmunization. SE has a higher level of the refractory and the average increase in the number of corrected lower after transfusion compared with the US. Our study suggests that further studies to evaluate the utility SPP HLA-matched at HLA-I alloimmunized LT patients is warranted.
